PMT Team Information Sheet Liability Waiver 2023

 

CHECK-IN VIA GOTSPORT

  • Deadline: Monday, May 15, 2023 by 10:00 PM NO EXCEPTIONS

Required Documents:

• Teams need to provide:

      • A copy of their State Approved Roster or their US Club Approved Roster. Please cross out any players not attending the event and write-in any guest players max of 3. Club pass players do not count towards your guest player limit. Players may only play for one team in the tournament and that must be their primary rostered team.
        • Please hand write their full name, jersey number, player identification number and birthday below the existing rostered players.
        • A copy of each US Youth Soccer Player Pass or each US Club Players Pass. Please cross out any player cards of players not attending
          • Only copy the front side of the cards if double-sided- with the player name, picture, and date of birth showing.
          • Include guest player cards for each guest player participating.
          • US Club Soccer teams may only take guest players registered under US Club Soccer, and USYS teams may only take guest players registered with properly stamped USYS player pass cards.
          • You must include coaches and manager cards as well.
          •  Print and Complete the Potomac Memorial Tournament Team Information Sheet and Liability Waiver (See Forms and Documents page).
          • USYS Teams outside of the East Region please submit an approved Permission to Travel Form from your state association
          • If your team is USYS registered and is from the East Region (ME, NH, VT, MA, RI, PAE, PAW, NYE, NYW, NJ, CT, MD, DE, VA, WV) a permission to travel form is not required.

Submission Process

• After you have gathered the required documents to register your team please scan or create PDF files for each document type as they will be uploaded separately.

  • Ensure you use descriptive names so you will know which files to send.
  • Each document will need to fit on separate PDF files for uploading purposes
  • Upload the documents into your GotSport Event Registration
    1. Log in to your coach/manager account and click on the “Team Management” button.
    2. Click on the name of the team you are working on.
    3. Open the “Registrations” tab.
    4. Click on the Event that you are working to Upload Documents to.
    5. Open the “Registration” tab.
    6. Next, click on the “Edit” button.
    7. For any files being asked to be uploaded, you can click on the “choose file” button and upload accordingly. If you have difficulty uploading a document, there is a good chance that the document is too large and will need to be decreased in size.

Approval Process

All teams will receive an email confirming that your team is checked-in and information is complete. Your team is not approved until you receive an approval email. Your tournament stamped approved roster will be uploaded into your GotSport event application. Our registrars will email the contact in your GotSport application by Monday, May 22, 2023, at 10:00 p.m.

Registration FAQ
Players not attending the event:
If a player is not attending the event do I leave their name on the roster?
No, if a player is not attending the event, you need to cross their name off of the roster, preferably in a thick, black permanent marker.

What is the maximum number of guest players a team can bring to the event?

No team may have more than 3 guest players. Please note club pass player are not considered guest players.

For US Club Soccer teams ONLY:
• A copy of each US Club Player Pass for guest players (front & back must be copied)
For US Youth Soccer teams ONLY:
• A copy of each US Youth Soccer Player Pass for guest players

All other relevant registration materials listed above must also be provided for EACH guest player.
The following are the maximum roster size per age group:
U09 – U10 (7v7) – 14 players U11-U12 (9v9) – 16 players U13 – U19 (11v11) – 22 Players, but only 18 may dress for any given game.

How do I add a guest player?
Step 1: Obtain the guest player’s current player card and medical release form
Step 2: Photocopy your original roster. Please handwrite the guest player’s full name, unique jersey number, player identification number, and birthday below the bottom of the existing rostered players. Please make sure the guest player’s current card is from
the same carding authority as the team’s official roster i.e. teams playing on a US Club roster may only have guest players with a US Club player pass and teams playing on a USYS roster may only have guest player with a USYS player pass. No exceptions!!

Medical Release Forms Do I include the medical release forms in the on-site registration packet?

All teams must have a medical release form for each player (including guest players) but DO NOT include the medical release forms in the registration packet.

If you are a USYS team and DO NOT have a medical release form please fill one out and keep it with you for the duration of the tournament. If you need a medical release, please use the USYS medical release form found here.
